Services
Social
Work Services are provided at the Vineland Community Nursing Service
office assisting patients and family members to adjust to their
respective illnesses and provide counseling for individual and or
family concerns. Services may also link individuals, with community
resources and facilitate placement into extended care or assisted
living facilities, or initiate referrals to support groups or to
mental health services.
One
of the primary roles of the Social Worker at VCNS is to provide
direct care to individuals/families for "social or emotional
problems that are or are expected to be an impediment to the effective
treatment of the patient's medical condition or his or her rate
of recovery."
Some
of the areas requiring Social Work intervention for some of our
home care patients at VCNS are as follows: financial resources to
maintain compliance with medical regimen (medications), assistance
with activities of daily living after discharge (bathing, dressing,
meal preparation), transportation to/from medical appointments,
assistance in the event of a bad fall (emergency response system),
depression, and caregiver burnout.
Social
Workers assist in referring individuals/family members to appropriate
local community agencies that provide specific services, that are
not provided by our agency.
